CMD Application Process

You can download your CMD applications from the links below. If you do not have Microsoft Word, you will need to download the PDF file version to print and complete.

If you choose not to download your forms, you may contact AMDCP staff at 410-992-3140 or via e-mail at cmdcoordinator@amda.com, to request a hard copy of the application be sent via U.S. Mail. The AMDCP board of directors reviews certification applications twice each year. Due dates are April 1 and October 1.
