CALTCM is proud to present our 34th Annual Meeting, Evidence-Based Care: Quality in Motion. This two-day conference highlights evidence-based care and quality improvement processes aimed at enhancing the quality of care in long-term care settings. Topics include risk management, urinary incontinence management, nutritional care, falls prevention and quality improvement, in addition to clinical updates on specific conditions.

This years meeting program will educate clinicians on quality and performance improvement methodology in a practical, straightforward, and clinically relevant manner. The material is relevant and valuable to physicians, nurses, nurse practitioners, physician assistants, pharmacists, administrators, social workers, and therapists. Highlights will include an overview of the updated F-tags for 2007-08 from the Chief Medical Consultant of the California Department of Public Health (CDPH), and the latest information on patient safety from the Agency for Healthcare Research and Quality (AHRQ). We are also very pleased to present a session on integrating cultural and linguistic competency into practice by Alecia Robinson, MCP, MPH from the Institute of Medical Quality.

Course Objectives

1. Describe recent advances and updates in diagnosis and treatment of common conditions in long-term care such as pneumonia, stroke, urinary incontinence, and weight loss.

2. Integrate performance improvement measures into clinical practice and nursing home care as promoted by Institute for Healthcare Improvement.

3. Identify recent clinical updates in focused areas of long-term care such as geropsychiatry, pneumonia, and neurological disorders.

4. Appreciate the changes in CMS Guidance to Surveyors on F-tags updated in 2007 and early 2008, including CDPHs latest strategies in monitoring compliance.

5. Implement strategies to enhance cultural and linguistic competency.

6. Examine methods of reducing risk of adverse clinical outcomes and medicolegal liability when providing care to residents in long-term care settings.

7. Implement and utilize care transition action plans in your own long-term setting.
